Progesterone in Men: What Your Level Means

Progesterone is low in men by normal physiology, not by deficiency. No major guideline body publishes a male target range to aim for. Here is what your number means and the narrow set of situations where it is worth investigating.

What progesterone measures in men

Progesterone is a steroid hormone produced in men by the adrenal glands and in small amounts by the testes. Its main role in male physiology is as a precursor: the body converts it along the pathways that produce cortisol and androgens. It is not a hormone with an established target level in men.

Why a low result is the expected result

Male progesterone sits at roughly the level seen in women during the follicular phase, which is the low point of the female cycle. A result near the bottom of the reference range is the normal finding. The clinically interesting direction in men is a high result, not a low one.

How to read a male progesterone result

Two things make this marker easy to misread: the reference range is assay-dependent, and the marker most often confused with it answers a different question.

The marker for adrenal enzyme disorders
17-hydroxyprogesterone, not progesterone, is the marker used to screen for congenital adrenal hyperplasia. The two names look similar and answer different questions.

Guideline bodies publishing a male target
The Endocrine Society male reproductive endocrinology guidance addresses testosterone, LH, FSH, and prolactin. We found no major body that publishes an optimal male progesterone level.

Progesterone is rarely ordered on its own in men. It usually appears because it was bundled into a broad hormone panel, and the result then needs interpreting without a clinical question behind it. That is the situation most people reading this page are in.

A result at or near the bottom of the range needs no action. Male physiology produces little progesterone, and there is no established deficiency state to correct. If a panel flags it as "low" against a range built from mixed-sex data, the flag reflects the range, not your endocrinology.

A result above the laboratory range is the finding worth following up. In men this most often traces to an exogenous source such as a progestin-containing medication, or to human chorionic gonadotropin prescribed alongside testosterone replacement, which stimulates testicular steroid production. Adrenal causes are less common and are investigated with 17-hydroxyprogesterone and cortisol rather than progesterone alone.

What is a normal progesterone level in men?

Should men take progesterone supplements?
This is not a supported treatment. Clinics that offer male progesterone "optimisation" are treating a number rather than a recognised condition. What causes high progesterone in men?
The most common cause is exogenous: a progestin-containing medication, or human chorionic gonadotropin (hCG) prescribed alongside testosterone replacement, which stimulates testicular steroid production. Less common causes include adrenal tumours and adrenal enzyme disorders. Adrenal causes are investigated with 17-hydroxyprogesterone and cortisol rather than with progesterone alone.
What is the difference between progesterone and 17-hydroxyprogesterone?
They are different molecules answering different questions. 17-hydroxyprogesterone (17-OHP) is the screening marker for congenital adrenal hyperplasia, most commonly 21-hydroxylase deficiency, where it accumulates because the enzyme converting it is impaired. Progesterone is not that marker. If your clinician is investigating an adrenal enzyme disorder, 17-OHP is the test that answers it.
Does progesterone lower testosterone in men?
Progestins used at pharmacological doses suppress gonadotropins and therefore lower testosterone, which is the mechanism behind their use in male hormonal contraception research and in androgen suppression. This describes administered progestins at therapeutic doses. It does not mean that a naturally occurring progesterone level at the top of your laboratory range is suppressing your testosterone.

Symptoms attributed to progesterone in men

These are the symptoms marketed as progesterone-related. Each is non-specific, and each has a better-established explanation worth ruling out first.

Low libido
Attributed to progesterone by hormone clinics. Total and free testosterone, SHBG, and prolactin carry established male reference standards and explain this symptom far more often.
Fatigue
Among the least specific symptoms in medicine. Ferritin, thyroid function, vitamin D, B12, and fasting glucose account for most cases with a blood-test explanation.
Poor sleep
Progesterone metabolites act on GABA-A receptors, which is the mechanism cited in marketing. That pharmacology describes administered progesterone, not variation within the normal male range.
Mood change and irritability
Testosterone, thyroid function, and vitamin D have clearer links to mood in men. A progesterone result within the male range does not explain a mood change.
Weight gain around the chest
Gynaecomastia in men is assessed through the testosterone-to-estradiol relationship, with prolactin and liver function alongside it. Progesterone is not the marker that answers this.

The most common explanation. Progestins appear in some treatments for benign prostatic hyperplasia, in appetite stimulants such as megestrol, and in compounded hormone preparations. Some immunoassays also cross-react with progestins, raising the reported number.

hCG alongside testosterone replacement
Human chorionic gonadotropin is co-prescribed with TRT to preserve testicular function. It stimulates testicular steroidogenesis, which raises progesterone as an intermediate. Assay interference
Immunoassays can cross-react with structurally similar steroids and with biotin at supplement-level doses. An isolated unexpected result is worth repeating, ideally by mass spectrometry, before it drives an investigation.

Adrenal tumours and enzyme deficiencies can raise progesterone, though these are uncommon and rarely present as an isolated raised progesterone. Investigation uses 17-hydroxyprogesterone, cortisol, ACTH, and imaging rather than progesterone alone.

Rare, and typically accompanied by other abnormal hormone results, gynaecomastia, or a palpable mass. A raised progesterone in an otherwise well man with a normal examination is far more likely to be medication or assay-related.

What to do with a progesterone result

For most men reading this, a low result needs no correction on its own. These steps cover what to check, and when it's worth a conversation with your GP.

Check whether the flag used a male reference range
Broad hormone panels sometimes compare male results against ranges derived from mixed-sex or female data, which makes a normal male progesterone look low. Confirm the range printed on your report is the adult male range before treating the flag as a finding.

A low result is expected, but raise it with your GP if you're unsure
There is no established male progesterone deficiency and no guideline-endorsed target, so a low result in a man is the expected physiology rather than a finding to correct on your own. If you're unsure why it was tested or what it means alongside your other results, it's worth talking to your GP or doctor. If a clinic proposes treating it, ask which guideline defines the target they are treating you toward.

For a high result, review medications first
List every prescription, over-the-counter product, and supplement, including biotin, which interferes with several immunoassays. Progestins and hCG explain most raised male progesterone results, and identifying one can end the investigation there.

Repeat an unexplained high result before investigating
Assay interference and laboratory error are more common than adrenal tumours. A repeat, ideally by liquid chromatography-mass spectrometry, confirms the result is real before it triggers imaging or a specialist referral.

Ask for 17-OHP if an adrenal cause is being considered
If your clinician is investigating an adrenal enzyme disorder, 17-hydroxyprogesterone is the marker that answers it, read alongside cortisol and ACTH. Progesterone alone does not settle that question.
What to expect after a change
A progesterone rise driven by a progestin or by hCG falls once the drug is stopped or the dose changes, on a timescale set by the drug rather than by progesterone itself. There is no timeline for "raising" male progesterone, because raising it is not a recognised treatment goal.
