Why your watch's blood pressure reading never quite matches your cuff

A wrist-worn device estimates your blood pressure from a pulse waveform. A cuff measures it by physically stopping and restarting blood flow. Those are different jobs, and the gap between them is wide enough that the FDA is only now, in 2026, writing a formal accuracy standard for the wearable side.

A cuff and a watch are not measuring blood pressure the same way. A cuff inflates, stops blood flow in the artery, and measures the pressure at which flow starts and stops again as it deflates. That is a direct mechanical measurement. A cuffless wearable instead reads a pulse waveform, usually optically at the wrist, sometimes via tonometry, and runs it through a model that estimates what a cuff would have shown. The watch is inferring a number from a proxy signal, not measuring the thing itself, which is the same shape of gap FixFirst has covered before with wearables estimating insulin resistance from wrist sensors instead of a blood draw.

There is not yet a finished accuracy bar for the wearable side. On January 23, 2026, the FDA released draft guidance proposing the first formal clinical performance standard for cuffless blood pressure devices: a mean error within plus-or-minus 5.0 mmHg and a standard deviation under 8.0 mmHg across validation testing, with a minimum 85-participant study population. It is still a draft, open for public comment through March 24, 2026. Until something like it is finalized, cuffless devices are not held to a single, agreed accuracy target the way a cuff-based monitor already is.

The regulatory fight already played out once, over labeling rather than accuracy. In July 2025 the FDA sent Whoop a warning letter over its Blood Pressure Insights feature, arguing that estimating blood pressure is a medical device function regardless of what the company calls it, and that "an erroneously low or high blood pressure reading can have significant consequences for the user." Whoop pushed back, arguing the feature was a wellness insight, not a diagnostic reading. The FDA closed the letter in June 2026 after Whoop adjusted its product and labeling to align with the agency's general wellness framework. Worth noticing what that resolution did and did not settle: it settled what Whoop is allowed to call the feature. It said nothing about how close the feature's numbers run to a cuff.

This matches what people are actually noticing. On r/QuantifiedSelf, one of the top comments in a recent thread on the topic put it plainly: "absolute numbers from wrist bp never matched my cuff. trends sometimes lined up after a boring week of baseline though." That is a reasonable summary of where the technology actually stands: single readings are unreliable against a cuff, but a longer trend line, once the device has enough of your own data to calibrate against, can be a directionally useful pattern to watch.

What to actually do with the number. Treat a cuffless reading as a prompt, not a result. If your watch flags a change, the useful next step is a real cuff reading, ideally the kind validated against a clinical standard, or bloodwork that touches the same cardiovascular picture from a different angle. A flag is worth acting on by checking it properly. It is not yet worth trending on its own as if it were the checked number.
