Your watch guesses insulin resistance. Should you trust it?

Google's Pixel Watch 5 estimates insulin resistance from wrist sensors, no blood draw required. It is a real screening signal, and it is also not the same thing as the test that diagnoses insulin resistance.

Google shipped a wearable that estimates insulin resistance without a blood draw. The Pixel Watch 5's Health Guardian feature, announced August 13, uses sensor data collected over time to flag patterns consistent with insulin resistance, alongside continuous blood pressure trend tracking. Coverage from Bloomberg and Financial Post called it a category first for a mainstream smartwatch.

The watch is inferring, not measuring. Insulin resistance is a metabolic state where cells stop responding normally to insulin, and it is diagnosed from insulin and glucose levels in blood, most directly with a fasting insulin test or a calculated HOMA-IR score. A wrist sensor cannot see either of those. What it can track is proxies, like heart rate variability and skin temperature patterns, that correlate with insulin resistance in some studies. Correlation is not the same as measurement.

That gap matters for what you do next. A "your patterns suggest elevated risk" notification is a prompt to act, not a result to file away. If your Pixel Watch flags this, the actionable next step is a fasting insulin test or an HbA1c, both of which give you an actual number against a reference range, not a risk pattern.

Early flagging still has real value. Insulin resistance often develops for years before fasting glucose moves out of the normal range, which is why standard annual bloodwork can miss it. A wearable that nudges someone toward getting tested earlier, before they would have otherwise asked, is a genuine screening use case, even with an unproven detection method.

The reverse case matters too. A watch that says nothing is not the same as a normal blood panel. If you have a family history of type 2 diabetes or other risk factors, get the blood test on its own schedule. Don't wait for a wearable to tell you to check.
