High Ferritin and High ALT: Why They Flag Together
Two markers from the same blood draw, both flagged high. Ferritin rises for two very different reasons — as a store of excess iron, or simply as an inflammation marker that goes up whenever the liver is under stress — which is why it so often moves together with ALT, the standard marker of liver-cell damage. The most common explanation by far is fatty liver disease, where a stressed liver raises both markers as a byproduct of the same underlying process. But a smaller, more consequential group of cases is a genuine iron-overload condition (hereditary hemochromatosis) that needs a different test to catch early, before it causes damage. How this combination happens
High ferritin and high ALT appearing together usually reflect one process driving both, not two unrelated problems. Ferritin is not just an iron-storage marker — it is also an acute-phase protein that rises with inflammation and cell injury, which is what a stressed liver produces alongside its own damage marker, ALT.
Four scenarios that produce this combination
The two flags are not always connected the same way. Here are the scenarios in order of likelihood, with the markers that distinguish each.
What does the AST:ALT ratio tell you?
Ferritin doesn't distinguish which scenario you're in nearly as well as the relationship between your two liver enzymes does. This one ratio is the fastest way to separate alcohol from the other three causes.
ALT and AST are both released when liver cells are damaged, but they come from different sources inside the cell and respond differently to different kinds of injury. In fatty liver disease and most other chronic liver stress, ALT typically runs higher than AST. In alcohol-related liver injury, the pattern classically flips: AST runs higher than ALT, often at a ratio above 2 — a finding described in the hepatology literature since the 1980s and still used as a bedside clue today (the mechanism involves alcohol's effect on mitochondrial enzyme release and B6-dependent ALT synthesis).
This ratio is a clue, not a diagnosis on its own — it can be less reliable once cirrhosis has developed, and some people with fatty liver disease still show a higher AST. But if your panel includes both ALT and AST and one is notably higher than the other, that single relationship narrows down which of the four scenarios above is most likely before you've done anything else.

Here's the counterintuitive part: both markers are usually silent until the underlying condition is well advanced. Feeling fine does not rule out either fatty liver or early iron overload — which is why this combination is so often caught on a routine panel rather than because of symptoms.

What to check first — and how
The right next test depends on which scenario you're most likely in. Transferrin saturation is the single fastest way to rule the highest-priority scenario in or out.

Transferrin saturation is the fastest way to separate hereditary hemochromatosis from the other three scenarios. If it's 45% or higher, AASLD guidance recommends HFE genetic testing to confirm or rule out the condition — this is the scenario most worth catching early, since iron overload is treatable and largely preventable with regular phlebotomy once caught before organ damage occurs.
If iron overload is ruled out, compare your AST and ALT directly. AST higher than ALT (especially a ratio above 2) points toward alcohol-related injury; ALT higher than AST, alongside elevated glucose or triglycerides, points toward fatty liver disease as the more likely explanation.
Fatty liver disease responds to the same interventions that improve insulin resistance — weight loss (even modest, 5–10% of body weight, has evidence behind it), reduced alcohol and added sugar, and increased activity. There's no medication that directly lowers ALT or ferritin in this scenario; both improve as a downstream result of the metabolic picture improving, typically over months, not weeks.
An ultrasound can confirm fatty liver directly and rule out other structural causes. If ALT remains elevated beyond 3 months of lifestyle changes despite a normal transferrin saturation and a fatty-liver-consistent AST:ALT ratio, that's the point to involve a doctor rather than continue self-monitoring.
