Why "Normal" Depends on Your Age and Sex

The same hemoglobin number can be a red flag for one person and unremarkable for another. It's not inconsistency — several markers have genuinely different guideline-published ranges for men and women. Here's the evidence behind each one.

Reference ranges are built from population data, and for a handful of markers, "the population" behaves differently enough by sex that a single shared range misclassifies real people. This isn't a footnote — the World Health Organization, the ACC/AHA, and NICE all publish sex-specific cutoffs for some of the most commonly ordered markers on a routine panel, precisely because using one range for everyone produces wrong answers in a predictable, biased direction.

The markers where this matters most aren't the obvious ones. Sex hormones like testosterone and estradiol are expected to differ by sex — nobody's surprised by that. The more consequential cases are the markers that look sex-neutral on the page: hemoglobin, HDL cholesterol, ferritin, liver enzymes, and uric acid all carry a physiological reason to differ by sex, and all of them appear on routine CBC, lipid, and metabolic panels most people get at least once a year.

Each of these has a specific physiological driver behind the sex difference — not an arbitrary convention.

Hemoglobin

HDL cholesterol

Estrogen raises HDL, so the guideline sets a higher minimum "protective" threshold for women than for men. Applying the male threshold to a woman's result would understate her actual cardiovascular risk profile.

Ferritin

Differences in average muscle and liver mass between men and women shift the typical enzyme range. Using a unisex range risks missing a genuinely elevated ALT in women, since the male-skewed range sets the bar too high.

Uric acid

Estrogen has a uricosuric effect — it helps the kidneys clear uric acid — so premenopausal women typically run lower than men. The gap narrows after menopause as estrogen declines.

NT-proBNP (age, not sex)

European Society of Cardiology heart-failure guidance
This heart-failure marker rises naturally with age even in people with healthy hearts. ESC guidance publishes age-banded cutoffs so an older adult's higher baseline isn't mistaken for cardiac strain.

FAQ — age, sex, and your reference range

Why does FixFirst ask for my age and sex?
Because several markers have different guideline-published normal ranges for men and women, and at least one shifts with age too. Hemoglobin, HDL cholesterol, ferritin, ALT, AST, and uric acid are all sex-specific by underlying physiology, not by convention — using the wrong threshold for your sex can misclassify a genuinely normal result as abnormal, or the reverse.
Why is the normal hemoglobin range different for men and women?
The World Health Organization publishes separate hemoglobin cutoffs for anaemia in men and women, reflecting real physiological differences: testosterone stimulates red blood cell production, giving men a higher baseline, while regular menstrual blood loss lowers the typical range for women before menopause. A hemoglobin level that's borderline-low for a man can sit comfortably in a woman's normal range, and vice versa.
Why do men and women have different HDL cholesterol targets?
Estrogen raises HDL, which is why the ACC/AHA cholesterol guideline uses a higher minimum HDL threshold for women than for men. A woman's HDL result needs to clear a higher bar to count as protective — applying the male threshold to a female result would understate her actual cardiovascular risk profile.
Why is ferritin usually lower in women?
Ferritin reflects the body's stored iron, and regular menstrual blood loss depletes those stores in a way that doesn't apply to men. NICE and other guidelines publish a lower ferritin reference floor for premenopausal women as a result. A ferritin level that would be concerning in a man can be within the expected range for a woman, particularly before menopause.
Does age change any blood test thresholds the same way sex does?
Yes, for at least one marker: NT-proBNP, a heart-failure marker, rises naturally with age even in people with healthy hearts, and European Society of Cardiology guidance publishes age-banded cutoffs to account for it. A small number of hormone markers (like DHEA-S) require both age and sex context together. Age affects interpretation more broadly too — the same marginally abnormal kidney or thyroid result carries different weight in a 30-year-old versus a 70-year-old — but that's a matter of clinical judgment layered on top of the guideline threshold, not a change to the threshold itself.
What happens if I skip the age or sex question?
