The markers most affected are BUN, the BUN:creatinine ratio, sodium, hematocrit and hemoglobin, total protein, and osmolality. How to tell dehydration from a real problem

Three patterns separate a hydration artifact from a genuine abnormal result.

Check the BUN:creatinine ratio first

A ratio above roughly 20:1 with a normal or only mildly elevated creatinine is the clearest dehydration signature. If creatinine itself is significantly elevated, look for a kidney explanation instead.

See if multiple markers moved together

Hemoconcentration raises BUN, sodium, hematocrit, and protein together, because they're all being measured in the same shrunk fluid volume. One marker high and the rest normal points away from a hydration cause.

Retest after rehydrating

The definitive test: drink normally for a day or two, retest under the same conditions (time of day, fasting status), and see if the values fall back into range. If they don't move, dehydration wasn't the driver.

Severe or clinically significant dehydration — from vomiting, diarrhea, heat illness, or inadequate fluid intake over days — is a different situation from mild under-hydration before a routine draw. It produces larger derangements (in sodium and osmolality especially) and needs medical evaluation rather than a simple retest. How to prepare so hydration doesn't skew your result

A simple, low-effort routine prevents most hydration-related false positives.

Drink water normally in the 24 hours before your draw, and have a glass or two of plain water in the 1–2 hours beforehand if your test doesn't require fasting. There's no benefit to overhydrating right before a draw either; extreme overhydration can dilute values in the opposite direction and, in rare cases, cause a temporarily low sodium reading.

Pale, straw-colored urine is a reasonable practical gauge that you're adequately hydrated going in — darker urine suggests you should drink more before the appointment if timing allows. If a test requires fasting (glucose, lipids), you can still drink plain water; fasting rules apply to food and caloric drinks, not water.
