Why the same low result means different things on different diets
Diet doesn't move the guideline threshold for any marker. It changes how likely a given result is to be a predictable dietary gap versus something that needs a closer look.
Diet is one of the clearest examples of why the same abnormal number can mean two different things. Vitamin B12 is synthesised by bacteria and occurs naturally only in animal products — meat, fish, eggs, and dairy. A diet that excludes all of these supplies zero natural B12, full stop. Low B12 on a plant-based diet isn't a surprising finding; it's the expected outcome of a known nutritional gap, and it's straightforward to close with a fortified food or a B12 supplement.
The same low B12 result on a standard, omnivorous diet tells a different story. If someone is eating meat, eggs, and dairy regularly and still running low, diet isn't the likely explanation — something is interfering with absorption instead, commonly pernicious anaemia, long-term proton-pump-inhibitor use, or metformin, all of which reduce B12 uptake regardless of intake. That's a different, more investigative path than "add a supplement."
Iron works the same way, for a different reason: it's not about whether the nutrient is present, but how well it's absorbed. Heme iron, from meat, fish, and poultry, is absorbed through a dedicated pathway at roughly 15-35%. Non-heme iron, the only form available in plant foods, absorbs at roughly 2-20%, and that rate drops further in the presence of phytates (found in grains and legumes) unless paired with a vitamin-C source. A plant-based diet doesn't guarantee iron deficiency, but it removes a margin of safety a meat-eater has by default.
